Triage Nurse Practitioner

We have a full time Advanced Nurse Practitioner at the surgery. Mrs Sue Cantrill. It is an ‘Emergency on the day clinic’, most mornings. She is able to prescribe. Contact the surgery on the day for an appointment. Appointments are not taken for advance bookings. Monday's are an early morning clinic starting at 7.00am. Other days clinics start at 8.00am.

Healthcare Assistant/Phlebotomist

We have a Health Care Assistants & Phelbotomist. A phlebotomy clinic is held every morning to take bloods. We do an early morning clinic for the taking of blood every Monday. The 1st appointment being 7.0 consultation2 0am. The Healthcare Assistant can do the following procedures in clinic by appointment.

Blood tests
Blood Pressure monitoring
Health Checks/Well Person Checks
ECG
Spirometery
Smoking Cessation Clinics

DIABETIC CLINIC
We have a special service for diabetics, held regularly by our Practice Nurse Team.

FAMILY PLANNING CLINIC
Our Practice Nurse is available for family planning and all contraceptive advice. Please book an appointment with the practice nurse clinic. Appointments are available daily 9.00am – 12.00pm and 4.00pm – 6.00pm

Dr Reshmi Dickson also holds a specialist clinic for coil fits and implant fits twice a month. If you are interested in this method of contraception, make an appointment with Dr Reshmi Dickson to discuss it further.

HRT (Hormone Replacement Therapy)
This clinic monitors the progress of women who are already on HRT and gives information to women who may be considering this type of treatment. Appointments are available daily 9.00am – 12.00pm and 4.00pm – 6.00pm with the Practice Nurse.

CERVICAL SMEAR CLINIC
The practice nurse carries out cervical smears. You will receive a letter directly from the Health Authority when you are due for this asking you to contact the surgery to make an appointment. Please make an appointment in the Practice Nurse Clinic. Appointments are available daily 9.00am – 12.00 and 4.00pm – 6.00pm If you are concerned that you have missed your appointment, please contact the surgery reception for clarification.

BLOOD PRESSURE CLINIC
Our Health Care Assistants are available for blood pressure measurement and monitoring. Please book an appointment with the Health Care Assistant in clinic. Appointments are available daily in both the morning and afternoon.

ANTENATAL CLINIC Child_belly
For the care of expectant mothers, the community midwives are available at the surgery. Ant-natal appointments are made directly with your midwife by contacting Eastwood Health Centre on Tel: 01773 712218. If you need to contact a midwife outside of normal surgery hours please ring : 01773 712218. Postnatal examinations are undertaken by the doctor. An appointment will be sent for this.

TEENAGE HEALTH PROMOTION
The practice nurse will discuss with patients any problems relating to the teenage years, including any general health advice and we ensure immunisations are up to date. The surgery will automatically send a letter to the patient around the 14th birthday.

MINOR SURGERY & CRYOTHERAPY
Minor surgical procedures, such as removing skin cysts and moles etc, are carried out by the doctors or Nurse Practitioner by prior agreement with the doctor.

CHILD HEALTH CLINIC / HEALTH VISITORS girl_with_open_mouth
Screening for health problems, especially in growth and development, in children under 5 years of age will be undertaken by Health Visitors who are based at ‘Surestart Children’s Centre’, Chewton Street, Eastwood. To book an appointment phone: 01773 712218

CHILD IMMUNISATION
These will usually be carried out by the practice nurse. An appointment is required within usual surgery hours.

DISTRICT NURSES
District nurses are based at Eastwood Health Centre. Telephone 01773 712218.

CHIROPODY
The chiropodist is based in Heanor. Appointments are made by contacting the clinic Tel: 01773 713149

PHYSIOTHERAPY
Appointments are made via referral by your GP.

ACUPUNCTURE
Dr Lim and Sue Cantrill our Advanced Nurse Practitioner have acupuncture clinics. An appointment with them in normal surgery to discuss if it is appropriate for you is required prior to booking in.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following: health_clipboard

Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
Insurance claim forms
Passport signing
Prescriptions for taking medication abroad
Private sick notes
Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Noticeboard

On Friday 15^th March staff at Church Walk Surgery raised a grand total of £384.84 for Comic Relief by selling cakes that staff had made.
We had a stall in the waiting room and cakes were sold to patients and visitors on the day.

APPOINTMENTS
For non-urgent appointments, please telephone or call into the surgery. Please let us know as soon as possible if you cannot keep an appointment so that we may offer it to another patient who may need seeing.

EMERGENCY FACILITY
If you require to be seen urgently, please tell the receptionist the nature of your request to assess the urgency. This facility is for emergency cases only and we ask you to not use this service for routine problems.

HOME VISITS
These should be reserved for patients who are genuinely too ill to attend surgery. Please telephone before 10.00am and be prepared to answer a few questions to help the doctors plan their calls and to give the doctors an idea of urgency. Please remember the doctor can see at least four patients in surgery in the time taken to do one visit.
